You can request for a lab test to be done at Disha Pathology Services by simply filling out the form below. We will contact you and schedule your lab test at your convenience. You can also request for a home visit, which will carry an additional charge.

Reports can be sent by fax, courier (please provide a complete and accurate mailing address) or email (please provide a correct email id).
